Application for registration of a birth occurring within 3 months. Submit at the Divisional Secretariat or hospital where the birth occurred.

Application for late registration of a birth — B6. Requires GN report and supporting evidence.

Application for a permit to fell timber. Requires GN certification of land ownership, site plan, and NIC. Submit at this DS office.

Application for a permit to transport timber. Requires GN certification of land ownership, site plan, and NIC. Submit at this DS office.

Application for a permit to extract sand, clay, gravel, or other minor minerals. Quantity and location must be specified. Subject to environmental clearance.
