Births · Marriages · Deaths — Registrations and Certificates
NIC applications (new issuance, renewal, and corrections) are submitted here. The DS office certifies and forwards the application to the Department of Registration of Persons. Both one-day and regular service options are available.
Passport applications are not handled at this office. They are processed directly by the Department of Immigration & Emigration. You can apply online or visit a regional passport office. A certified copy of your birth certificate from this office may be required as a supporting document.

Visit the ADR Division counter at Welipitiya DS within 3 months of the child's birth.

Submit the BHT 30 card issued by the hospital, or a declaration if born at home.

Complete the birth notification form with both parents' details and sign the declaration.

The first certified copy is issued on the same day, free of charge.
- BHT 30 card from hospital (or home birth declaration)
- NICs of both parents (originals & copies)
- Birth certificates of both parents
- Parents' marriage certificate
- Child's horoscope (for home births, if available)
Registration within 3 months is free of charge. The first certified copy is also free. Additional copies cost Rs. 100 each.

Submit Form B6 declaration to the ADR Division at Welipitiya DS.

3 months to 1 year: The ADR can approve and register directly at this office.

Over 1 year: Application is forwarded to the Registrar General's Department for central approval.

On approval, the birth is entered into the e-BMD system and a certificate is issued.
- Grama Niladhari certificate confirming birth details
- Hospital record or BHT 30 card (if available)
- Child's horoscope
- NICs & birth certificates of both parents
- Parents' marriage certificate
- School records or immunisation card (if applicable)
For persons over 21 years, the applicant must declare themselves. Registrations delayed by over 12 months are forwarded to the Registrar General's Department — this may take additional processing time.

Give marriage notice to the Marriage Registrar at Welipitiya DS at least 4 days before the intended date.

Notice is displayed for 10 days. If no objection is raised, the marriage may proceed.

Solemnise the marriage in the presence of the Marriage Registrar and two witnesses.

Marriage certificate (B121) is issued by the Registrar after registration.
- NIC / Passport of both parties (originals & copies)
- Birth certificates of both parties
- Divorce certificate or late spouse's death certificate (if previously married)
- Muslim marriages: Wali declaration (Form E2/3)
- Both parties must be at least 18 years of age
Kandyan marriages follow a separate procedure under the Kandyan Marriage and Divorce Act. Please enquire at the counter for specific guidance on Kandyan registrations.

Obtain the Medical Certificate of Cause of Death (BDR 26) from the attending physician or hospital.

For sudden or unnatural deaths, obtain the post-mortem report and police report first.

Submit BDR 26 and supporting documents to the ADR Division counter at Welipitiya DS.

Death is registered and a death certificate plus burial permit are issued.

- NIC of the informant (next of kin registering the death)
- Unnatural deaths: post-mortem report & police report
Deaths should be reported as soon as possible. The death certificate is required before burial and for pension, NIC cancellation, and estate purposes. Late registration requires a Grama Niladhari declaration and ADR approval.

Pay Rs. 100 per copy at the DS cashier and attach the receipt.

Submit at the ADR counter. Records in the e-BMD system are issued same day. Older records may take 1–3 working days.

The original certificate received at registration is not valid for Ministry of Foreign Affairs attestation. You must obtain a fresh certified extract from this office or the Registrar General's Department for visa and overseas applications.

Submit the relevant declaration form to the ADR Division at any DS office in the district.

Under 21: Declaration made by a parent or guardian — use Form B7 (adding name) or B9 (altering name).

21 or above: Person declares themselves — use Form B8 (adding name) or B10 (altering name).

ADR reviews and approves. The amended entry is recorded and a new certified copy is issued.
